Four design principles apply. First, maintain proficiency by institutionalizing blind reads in which radiologists interpret a fixed share of images independently before seeing the AI result and then compare. Second, make disagreement visible by always recording cases where the AI and physician diverge and analyzing them in regular case conferences. Third, design for cognitive load and time so that confirmation does not become an assembly line, with adequate time per case and caps on continuous work periods when attention degrades. Fourth, ensure explainability and clear accountability by displaying the AI's basis (regions of attention, confidence), keeping final judgment and sign-off with the physician, and documenting the scope of responsibility. Concrete mechanisms include a dashboard continuously comparing AI accuracy against independent physician reads, simulation training on rare diseases, and turning AI errors into teaching cases. These protect patient safety and physician expertise whether AI accuracy keeps improving in an optimistic scenario or degrades and misses novel conditions in a pessimistic one.
Several studies in the 2020s showed medical imaging AI matching or exceeding specialist accuracy in breast cancer screening and chest X-rays, supporting an optimistic scenario. Yet other findings show that human-AI collaboration does not always improve accuracy, with interaction problems such as physicians being pulled by AI errors (automation bias) or over-doubting the AI when it is right. Bainbridge's ironies of automation, the levels-of-automation model by Parasuraman and colleagues, and crew resource management (CRM) from aviation apply directly to this job design. A strong answer goes beyond saying humans will check, proposing concrete mechanisms across proficiency, learning from disagreement, attention limits, and accountability, while anticipating scenarios in which AI performance changes. It is even stronger if it reallocates time freed by AI to work only physicians can do, such as dialogue with patients and clinicians and review of complex cases.
Blind reads and case conferences take time and money and are easily seen as contradicting the efficiency goal of AI adoption. As physicians encounter AI results more often, their judgment may drift toward the AI unconsciously, so independence cannot be guaranteed by design alone.
Make the share of blind reads adaptive rather than fixed, adjusting it according to the disagreement rate between AI and physicians and the appearance of new case types. Agree at adoption that part of the efficiency gains will be reinvested in proficiency maintenance, and build in periodic measurement of physicians' independent accuracy with intensified training whenever decline appears.
